[Msoadm] Student Forms: Winter 2026 Undergraduate Enrollment Adjustment Form

Dear UCR Faculty,

In October 2025, we announced the launching of a new comprehensive *Student
Forms Portal* <http://studentforms.ucr.edu>*.* The Undergraduate Enrollment
Adjustment Form went live for Fall 2025.  For Winter 2026, the Enrollment
Adjustment Form will now incorporate the ability to select prior terms for
retroactive requests.

This, and future forms, will follow defined routing logic for required
actions and, when applicable, will be automated to update in Banner. This
will include direct engagement from you with the system for approvals, when
appropriate. Please check your email regularly for requests related to
Undergraduate Enrollment Adjustment Forms.

Please review attached references to the initial notification as well as
navigation screenshots that were sent out in October 2025.
