[Students] UC Flu Vaccine Executive Order/How to Comply/DUE 11/19/2021

Dear UCR students,

The UC Office of the President has issued a Flu Vaccination Executive Order<https://ucnet.universityofcalifornia.edu/coronavirus/student-faq-uc-2021-2022-flu-vaccination-executive-order.pdf> that states any student who accesses any UC campus and may be living, learning or working on any UC campus must comply with the 21-22 executive order by November 19, 2021. The 2021-2022 flu season began September 1, 2021 and ends May 31, 2022.

Please visit the Student Health Services (SHS) website at studenthealth.ucr.edu<https://studenthealth.ucr.edu/> for SHS flu vaccine clinic dates and times. To schedule an appointment call (951) 827-3031 and select Option 2 during business hours located on the website. UCSHIP is free, non UCSHIP cost is $30. Local pharmacies and your medical provider also have flu vaccines available.

How to comply by getting vaccinated:


  1.  Get your 21-22 influenza vaccine and record of the vaccine.
  2.  Log in to your SHS patient portal, go to medical clearances, find Influenza vaccine, enter the date, enter the type of flu vaccine found on your record and submit.
  3.  Go to "immunization record" and hit the UPDATE button. UPLOAD your flu immunization record in PDF or JPG format. Submit. Record must be submitted at the same time as date entry or your compliance will be reversed upon audit.

How to comply by declination form:



  1.  Log in to your patient portal.
  2.  Go to download/upload forms.
  3.  Find 21-22 flu vaccine declination form and download.
  4.  Read and comply with non-pharmaceutical interventions as outlined in executive order.
  5.  Complete by signing, dating and printing your name (PDF, JPG format accepted).
  6.  Log in to patient portal to download/upload forms/flu declination.
  7.  Hit the Upload button, check your form, upload your completed form, click submit.
  8.  The completed form UPLOADED will meet compliance; however, the system will not show compliant until your form is reviewed by staff. A declination exemption will be applied to your chart until May 31, 2022, if the form is complete. Please be patient during this process. You will be considered compliant during the processing period.
  9.  To access the SHS building from November 19, 2021, through May 31, 2022, you need to have a compliant 21-22 flu vaccine or completed declination form UPLOADED on file in your chart. If your appointment is for the flu vaccine that will allow temporary access. Please make sure this requirement is completed prior to your appointment. Please review the tutorial video.



THERE WILL BE NO REGISTRATION HOLD FOR INFLUENZA VACCINE for noncompliance. You are not allowed to access any UC campus after November 19, 2021, until you come in to compliance as outlined in the flu executive order.
To contact the immunization mandate staff in your patient portal, select messages. From there, select new message and choose Immunization Mandate Staff to send a message. Please allow 3-5 business days for a response during registration periods due to high volume.
